Getting Ready for Your Visit

In order for us to provide the best care for you, it is very important that you bring the following items to your visit:

Photo ID
A list of your current medications
Any and all x-rays films
Insurance information and card
Information about what treatments and medications have been tried in the past
Medical records from prior visits that you think may be helpful
New Patients: We offer automated pre-registration. Watch your email or text messages for a pre-registration link and fill out your forms from the comfort of your own home! If you prefer to print a paper copy, please download a copy of our form below.

Routine Vaccines are Recommended

We advocate that all patients receive the routine vaccines recommended by the Centers for Disease Control and Prevention (CDC) and the American Academy of Pediatrics (AAP). The recommended vaccines and the schedule by which they are given is the result of many years of scientific study and data gathering on millions of children by thousands of our biggest scientists and physicians.

We recognize that the choice to vaccinate may be difficult for some parents or caregivers. We will do everything we can to provide you with the information and answer your questions. Should you have doubts, you can discuss them with your healthcare provider.
